for the scenario #2, there is one more finding:

If the default locale is "de_DE", the Firefox shows two language settings "de_DE" and "de". By default the "de_DE" is selected. If switching to "de" the issue is gone. This has been verified in Firefox 3.6.6 on Windows.

To change the language setting in Firefox:

Version 3.0 and newer:

 Linux:
    Edit->Preference->Content->Languages->Choose...
 Windows:
    Tools->Options->Content->Languages->Choose...

Hope this finding gives more hints to the solution of the issue.
