Ronald, thanks for the tip.  I was able to get it to work.  One problem I do have is if I click the Generate Form button and type in field names as soon as I click or tab out of the field the name gets changed to "variable1", "variable2" etc?  So I can generate forms but not with any meaningful variable names?  If anyone knows how to correct this problem please let me know, thanks, Bill
