[keycloak-user] Keycloak Duo MFA SPI

I searched for an authentication integration with Duo Security and came
across the recommendation in this user list to write it as an Authenticator
SPI (
http://lists.jboss.org/pipermail/keycloak-user/2016-November/008234.html).
I have written one and just wanted to share it here:
