The reason for disabled caching is the calculation of unread topics in the topic list when a user is logged in. The list simply can't be cached, as the icon might change with every request and the logic for displaying the read/unread icon needs to execute on every request.
The underlying issue is MySQL which is extremely slow on moderately complex joins.
