  Removes the new server action from the Servers view toolbar

* Replaces the new server action with the refresh server action so it is
  more prominent

* TeiidServerActionProvider
 * No need to provide any other special actions on the toolbar when the
   teiid server configuration is selected

* plugin.xml
* RefreshServerAction
 * Adds the action as a command to the toolbar using extension framework
 * Converts action into a handler so it can be executed via the extension
   framework
