Added internal materialized views as a first class concept.  There is a new system table MatViews to query for state, expanded caching hint to control how they are created, and new system functions for refreshing.

There is still much more that can be done, but all of the basic support of of old approach is covered with some additional features.  The doc updates will be covered under TEIID-168

Likely TODO:
replication of the materialized view
lazy refresh
cascading refresh or the notion of a materialized view group
non primary key indexes
