Removed duplicate codes, but did not change the visit methods.  Will leave this open for now to further reduce the number of codes and improve the messages.

In both of those instances we should see if a unique code is needed or if it can actually be removed.  For comparison Postgresql use only about 250 error codes.  As we work TEIID-1386 it will be important to minimize the number of error codes.
