Added support for pg_client_encoding() which showed that at least for the jdbc client calling "set client_encoding ..." was ineffective unless a parametermessage was sent back.  So it's likely that non-init setting of client_encoding may not be working properly in older branches.

The changes here are quite large and contain several minor refactorings, which could be left out for a more targeted fix to older branches.
