I don't see anything immediately wrong with the wrong result plan. 

The inner most join does have the predicate "convert(adm.patientid, string)=l.admissionid" which is concerning (if there is a collation issue), but seems to return the expected rows in the grouping plan.  In addition to these debug logs, can you provide the query plans post execution that show the node row counts?
