There are different issues on different platforms that make the device not register w/ the UnifiedPush Server.
On APS some common issues are:

code signing issues (production vs. sandbox)
-
provisioning profile issues
-
incorrect URLs / credentials (from UPS)
-
Incorrect SenderID
etc.
The document generated by this issue should give advice on how to gather data and debug it, when issues w/ the device registration process occur.
Basically we do need something like a trouble shooting document
