If only the start time is specified, then we publish the page at the time and have the page exist until someone wants to change it. This would be the same as specifying an infinite end time or the same as if someone created the page at that time and didn't specify publication settings. If only the end time is specified, then we should publish the page immediately and remove it at the specified end time.
