Fix committed in svn (revision 2367).
There was a flag being used to determine if the nodes were already expanded or not, if the
flag specified that nodes we all expanded then it would ignore the request to expand the
nodes. There was an issue with the flag in that it was only being set as not expanded when
collapse all was used. It should have been set whenever any node collapse on the page
occurred.
Even if the flag was being selected properly, it would have still acted inconsistently
with the rest of the actions. When clicking collapse all, collapsing an individual node,
or expanding an individual node, then any updates to the nodes would also have appeared on
the page (ie if new nodes were added or removed). But if using the flag, then nothing
would have happened if the current nodes were already expanded and any node changes would
not have been seen.
The flag has now been removed, clicking expand all should try and expand all the nodes
everytime.

- Click cross icon (-) to hide sub page(node)
- Continue click Expand All --> nothing is happened --> Error
