The example and table numbering issue has been resolved by now. They are numbered individually for each chapter. The remaining issue is the style of cross-referencing (see the comment on GitHub linked by Hardy above). And this should actually be configurable now in AsciiDoc as per the last comments on the feature request. Would be great if someone could follow up on this and configure it accordingly in our build.
