I think is should have "Affected Version" set to 5.2.12.Final and the "verify-affects-5.0" label should be removed.

The provided test case runs with the 5.2.12 version, if "org.hibernate.ejb.HibernatePersistence" is replaces with "org.hibernate.jpa.HibernatePersistenceProvider" in the persistence.xml
