Regarding questions you posted, I am surprised why you say Hibernate does not use ehcache if asked to use as second level cache provider. As per the documentation it clearly states that it uses two cache regions and I see these two on my EhCache cache name list.
We have 4GB of HEAP allocated, we could increase but we want to make sure we are not just postponing the problem. The total heap dump size is more than 5GB and unfortunately i won't be able to share it. I am sharing few screenshots where it shows the severity of problem. We would be happy to upgrade hibernate but just trying to see if this exact problem is fixed in those. I have seen other reports where folks complained about 5.2 as well. In My earlier comment i shared a screen shot where over 5K query cache plans are on the heap taking more than 3GB of heap space.
