You will get the bpmn file from jbpm-installer/sample/evaluation/src/main/resources/Evaluation.bpmn

and ftl file from jbpm-installer/jboss-as-7.0.2.Final/standalone/deployments/jbpm-gwt-console-server.war then go to WEB-INF/lib and open the jar jbpm-gwt-form-5.2.0.Final.jar.
