This is/was a bug with Guvnor which is used in jBPM 5.2. Should be fixed once we pull the latest version in jBPM 5.3. Make sure you do not have spaces in the TaskName property value, so rename "Performance Evaluation" to "PerformanceEvaluation" in your process, regenerate forms, build your package and try again.
