Regarding General Issues.

The field was already renamed to "Name". (FIXED)

The "Name" select box lists the available deployments. You must select one in order to create both Identity Provider and Service Provider.

I'm not sure if we can use an input box here. The idea is always reference on of the available deployments.
Regarding Adding external IdP Issues.

I've fixed that. The "Url" field is now required when you check the "Is External" checkbox. (FIXED)

I don't think we need to do that. In this case, we just don't care about the name. Only the url is important.
