1. The default is that when encountering faults, the work item is still completed. The default should be changed to not complete, but allow it to be overridden by the (existing) CompleteAfterFault flag.

2. When a fault is encountered, it is silent. Instead, an ERROR should be logged, so users aren't left scratching their heads as to why nothing is happening.
