See the last comment, the design time name of the table would be the same as the effective
runtime name - catalog.schema.table. This is what happens on the runtime side anyway when
design time catalog/schemas are used. The actual runtime table name becomes
catalog.schema.table, and there is no runtime concept of a source level catalog/schema.
So we're really just proposing that designer mirror the runtime.
Catalog and Schema should use different icons so that they can be
distinquished as to their type
