Changed to default to "document", which is the default style, when style is not
present. Also, added logic to get the resolved element for the name when the name is null
on the XSDElementDefinition. Also, updated logic to exit schema loop when the element is
found in the getSchemaElementForSelectedOperation(). The logic was continuing and adding
the same element four times for this WSDL. Lastly, added XSDModelGroup nodes to the
SchemaTree to allow xpath to properly resolve.

A few issues here... the style was not set in the binding or the operation. This is
valid, but we were not defaulting to "document" which is the default. That's
why the message "The style for the service cannot be determined." was being
generated.
Also, the name was null on the XSDElementDeclaration. Added logic to get the resolved

Trying to import the wsdl jrl_inquirywsdl.wsdl (in attached zip) using the import
--> WSDL to Relational Model option. After selecting the wsdl from the file system,
then validating, the next panel presented the methods to select (none-preselected).
When you highlight a method, in the Select Details section it says:
"The style for the service cannot be determined. "
The next or finished button was never enabled.
