Resolution: Done
Per the screen caps, we removed the ability to create schema/catalog objects during
import. Added a fully-qualified name option that applies to both object names (table,
view, procedure, index) and Name In Source value.
Name In Source is now ALWAYS quoted if a quoted character exists. Previously, it only
quoted only if character exists, there was a naming conflict or the name contained an
"extra" character defined in the database metadata.
Added tooltips to new check-boxes.
Placed model info in it's own "group/panel" at top of page.
Catalog and Schema should use different icons so that they can be
distinquished as to their type

The catalog and schema should use different icons so that they can be distinguishable in
the tree. The mouse-over type would also be helpful.
