[~mkralik] Teiid-related connection profiles are contributed by Teiid Designer plugins.
Most of the JDBC connection profile contributions are provided by the Eclipse Data Tools
plugins, which are managed/fixed in Eclipse. So in this case, you'd need to file an
Eclipse Bugzilla issue for this SQL Server issue.

When I want to create a connection profile for SQL Server 2012 and 2014, after creating a
driver, connection profile doesn't show any labels for fill information about DB
(screenshot1). SQL Server 2008 works great (screenshot2).
I noticed that this happens when a user chooses SQL Server 2012 or 2014 driver template.
