I have created a test case that demonstrates the problems we are having, it is attached.  Without commenting out criteria.setMaxResults in QueryLoader, the test returns 1 out of the two expected results, and on that one result, the number of embedded objects is 2 out of the 4 that are added to the Set.  Commenting out the criteria.setMaxResults line causes the test to pass.
