I also found this same problem.  I looked at the implementations provided with this ticket but wasn't all that happy with the performance I got out of them, so I wrote a new one (see attached BindingSqlServerDialect.java).  This is now deployed in several of my production systems working against database tables with tens of millions of records.  It has GREATLY improved the system's performance and efficiency, especially under heavy load.  I certainly hope it is included as a part of the Hibernate codebase in the near future.
