The dtd found at http://hibernate.sourceforge.net/hibernate-mapping-3.0.dtd is does not specify a lazy propery for the <map-key-many-to-many> and <index-many-to-many>. As a result it is impossible to create a map which doesn't lazy-load it's key value. (default-lazy="false" does not help either.)

Using a custom dtd (see attached patch) the lazy property can be specified and works as expected.
