RuleBuildContext does not initialize package of rule

When the RuleBuildContext constructs a Query or Rule in its constructor, it does not pass in the package name as a parameter; consequently, Rule.getPackage() always returns null.

In a related note, Rule.equals() will always return the same value as Object.equals() (owing to the second line), unless the rule is compared with itself.  This makes it inconsistent with Rule.hashCode().
