TreeCacheProviderHook should get its cache via the JBC CacheManager rather than using JMX integration.

This class will be replaced by Hibernate 3.3 soon, but I want to do this anyway so I can get rid of the ejb3-entity-cache-service.xml and have all AS5 caches deployed via the CacheManager.
