You can add multiple conditions, actions, attributes and/or metadata, with the same name (or even duplicate of the same data) and they conflict with the way data is edited and deleted. Examples:

Add same condtion, or different conditions with the same name
Add the same attribute twice
Add 2 pieces of metadata with the same name

The simpliest fix is likely to make sure all header names are unique. My thought is there is a global way to prevent this, so I will leave this to the Mic.
