We need to extract the concern of binding into JNDI from the existing proxy-impl logic.

General idea:  Deployer to read in on metadata, install into MC some beans to @Start bind LinkRefs into EJB3.1 java:global pointing to our DefaultJndiPolicy stuff.

Also consider a chain of configured binders to bind all appropriate entries (ie. Our DefaultJndiPolicyBinder would be a dependency of the EJB31Binder, which makes LinkRefs).
