I'll have to run the server again and analyse the heap once more.  I had the full chain in an email a while back, but I can't find it now.

Even assuming "PER REQUEST", I didn't notice any code that was going to clear the thread local "PER REQUEST".  I only saw code that sets the variable.

I'll have more info tomorrow on this.
