A ManagedConnectionFactory doesn't have a JNDI name. Will put JNDI name into ConnectionFactory

Plus the Pool / PoolConfiguration also belongs to the ConnectionFactory, so

  CF
  - MCF
  - Pool
  - PoolConfiguration

Also will refactor rhq plugin code to match it
