Updated from discussion yesterday (20150908):

- XML changes deferred to later release.
- ignore-unused no longer has a default in XSD.
- If --backup is used then ignore-unused is false if it is unspecified in XML
- if --backup is not specified, then ignore-unused is true
- if --backup is used and ignore-unused is true in XML, then it remains true.
