[~gabriolo] K, I've looked at a version. The items you are raising weren't part of the design. The original design did not have an "Index" or "Description" header. Instead the starting point was intended to focus solely on the first step which was to create Given and Expect statements.

Likewise, there was not a concept of multiple header rows (multi instance.) So I need to double-back a little and ask you a couple of questions:
* If we're now providing an empty scenario row by default (initial view), do we even need contextual menus on the headers?
* Do we need the Index and Description headers at all? Instead the initial row could offer hint text in the cell:
