[~manstis] Thanks for clarifying - yes it would be contextual to the type of column header and table, sorry for not specifying. :)
* Question: Do we really need a new menu item "Append?" For consistency it might be nice to use the same language as is used in the respective row menu labels "Insert"...  Do you think it's a common case that they user will be adding a row to the bottom by clicking on the header? 
* WRT disabled, I just kept with the model that we're already use for the other menus.
