[~dadossan] thank you for a message. I can confirm I can not reproduce the issue anymore. I tried:
- Open existing model (Trafic_Violation), click into annotation column header
- Create new model, have multiple annotation columns, some named, some with empty hader, save and reopen

I think we can close this.
