Good call.  It was an old version of web.xml.  I was using 2.2:


  | <!-- !DOCTYPE web-app
  |     PUBLIC "-//Sun Microsystems, Inc.//DTD Web Application 2.2//EN"
  |     "http://java.sun.com/j2ee/dtds/web-app_2_2.dtd" -->
  | 
Switched to v2.4:

  | <web-app version="2.4"
  |         xmlns="http://java.sun.com/xml/ns/j2ee"
  |         xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance"
  |         xsi:schemaLocation="http://java.sun.com/xml/ns/j2ee
  |         http://java.sun.com/xml/ns/j2ee/web-app_2_4.xsd">
  | 
and it works as expected.
