Few things to note:

If you have a cluster setup, normally you would put a load balancer such as mod_jk in front so that one user has one session and session information is passed on each request. If you are directly hitting nodes on cluster and logging in as a same user, basically you are creating two sessions for one user and hence the error you are seeing.

But this error is only shown on server log. From what I have observed, portal web interface does not show anything.
