Hi,
I wondered how to display a form when we start a new instance of bpmn process?
in the sample included on jbpm 5.3 full installer, if we start new instance of evaluation process, the form as shown in the image below will be displayed.

I tried to make another bpmn process and deploy it to the guvnor repository, but when I try to start the new process instance in jbpm-console, no form is displayed. I even cannot display the diagram.

What should I add to the bpmn?
