make sure that jbpm-human-task.war application is properly deployed to your application server. When it is deployed it will log its status in server log. Unfortunately I can't see these status messages in attached server logs as they seemed to be incomplete. I would suggest to run ant clean.demo and ant install.demo once again and first verify if everything is working on default data base and next apply changes for mysql.
