Slava means the text below white wizard header 'Choose source folder and enter file name'. If look at http://www.eclipse.org/articles/Article-UI-Guidelines/Contents.html#Wizards it shhould be show as message in wizard title.

3a - corresponds the http://www.eclipse.org/articles/Article-UI-Guidelines/Contents.html#Capitalization 

"Guideline 1.6 Use Sentence style capitalization for all control labels in a dialog or window, including those for check boxes, radio buttons, group labels, and simple text fields.  Capitalize the first letter of the first word, and any proper names such as the word Java."

3b - could be "Reverse engineer from Database" if Database can be treated the same way as Java in Guidline 1.6, but I'm not sure and "Reverse engineer from database" looks good for me.
