Is this a new issue or did it just start happening?

The fact that there are no icons means something is very wrong with that build. I'm pretty sure we did not release a build in that state. I would be more than shocked, as I create new servers and runtimes every day. 

In my current workspace, it works for me.  I am able to create new servers and runtimes, the icons show, the wizard has "next" properly enabled. 

I feel pretty sure that with the Alpha out for so long, we'd have heard of such a thing happening weeks ago if not longer.  Not to mention our QA team would have DEFINITELY noticed this before release. 

I need to resolve this issue as works for me. I highly suggest re-installing as it seems like something has gone wonky in your installation.  The icons are provided via the extension point in as.u, as are the wizard pagesi.  It would seem to me as if perhaps your org.jboss.ide.eclipse.as.ui plugin is not present.
