When selection strarts from the source pane - visual pane corresponds to it perfectly.

When selection goes in the visual part - source part is ok, but when after that you'll start selecting in the source - visual part won't change.

After broken selection, when you keep selection on text in the source part and than you'll press visual refresh button - selection will be correctly restored in the visual part.
