Just encountered same issue. Problem is that there are two files in hibernatetools\docs\task-reference\en-US folder with name differing only in case: sect-Hibernate_Tools​_Reference_Guide-Con​cepts-Ant_Tasks-Hibe​rnate_Configurations​.xml and sect-Hibernate_Tools​_Reference_Guide-Con​cepts-Ant_tasks-Hibe​rnate_configurations​.xml, and that Windows unlike Linux is "case-preserving, but case-insensitive".
