the log entry you paste is especially interesting since it points to a sensible topic: older JBoss Tools would create an application and the wait for it to become ready via a "health" url:

If this happened when you tried to create your application and embed the catridge(s) then the wizard would have errored, the application would have been created but the cartridges not added. Is this what happened to you? 
I re-tried and could still not reproduce it unfortunately. All went fine: The creation wizard went all fine, when looking up the application in the OpenShift Explorer my application was listed and had my cartridge (in OpenShift Explorer and in "Details" dialog).
May I ask you kindly to try again since STG was re-deployed with the latest code yesterday? The above error is a pretty serious issue. If this happens the way I suspect it then many of our existing users (the ones using our 4.0.0.Final and below) would get broken.
