Issue resolved. Doc available for review on docs-devel and tagged live for customer portal.
For future ref, both xml files for chps 3 and 5 had the line {noformat}<?dbhtml filename="getting_started.html"?>{noformat}. This is used for chunking and the duplication caused the issue. It was resolved by changing wrong ref in chp3 file to {noformat}<?dbhtml filename="installation.html"?>{noformat}.
