As Michelle says, it's probably better to keep this first page as simple as possible.
But we could have an advanced page where user could set some values such as Affected Version (that would be set automaticall by inspecting the runtime). If it's a wizard, we can think of this very basic first page, from where user can click "Finish" or "Next >". If user click "Next >" then he sees some advanced fields.
