I'm not able to reprodue this. If a beforeCompletion throws an Exception it's caught in TwoPhaseCoordinator.beforeCompletion. AtomicAction.commit calls ThreadActionData.popAction() on the return path. You'd only get this problem if TwoPhaseCoordinator.end or something it calls throws an Exception that's not caught, causing  AtomicAction.commit  to be skipped. Which this stack does not indicate. I'm going to need better logs or a test case otherwise I'll close it as non-reproducible.
