I'm doing the QA of JBWS-1643 now and got the same error with org.jboss.test.ws.jaxrpc.samples.wsbpel.hello.BpelHelloTestCase
I've run it many times and no change. I then started the all server where the test also failed. Finally started the default instance again and the test passed...

JBoss-4.0.5_GA from svn
jdk1.5.0_11
jbossws-1.2.0.SP1 from svn

First I've run the the regular tests (with ant tests) and then without stopping the server the sample tests. Hope that helps.
