Filter mapping for ajax4jsf filter should be the first one in web.xml:

	<filter-mapping>
		<filter-name>ajax4jsf</filter-name>
		<servlet-name>Faces Servlet</servlet-name>
		<dispatcher>FORWARD</dispatcher>
		<dispatcher>REQUEST</dispatcher>
		<dispatcher>INCLUDE</dispatcher>
	</filter-mapping>

This should be removed as it shadows mapping for ajax4jsf filter:
	<filter-mapping>
		<filter-name>extensionsFilter</filter-name>
		<url-pattern>/*</url-pattern>
	</filter-mapping>
